Option 1:
For your first loan, use option 2, but for future loans here are simple options:
If you have authorized an account for electronic funds transfer you can:
- request loans under $5,000 over the phone - 1.800.243.6631 x1
- request loans up to $25,000 via your online account access
Requests above $25k need to be completed via paper form, option 2.
Option 2:
- Access the forms at the bottom of this article.
- Complete loan forms section at the top, policy number and name, section 5, and signatures. In the special instructions write in "deposit via EFT"
- Complete the EFT authorization and include a scanned copy of a voided check.
