How do I take a loan against my Mass Mutual policy?

Here are the simple steps:

• Access the forms attached below.
• Complete sections A,B,D,F on the loan application form and sections A, B, D on the EFT authorization form. Don't forget to include a voided check. 
• Email these forms to lifefax@massmutual.com for processing.
