Here are the simple steps:
- Access to most recent form via the attached document.
- Complete the top section, 2, and the signature fields on the second form. Include in the special instructions to deposit via EFT and include a scanned image of a voided check to the personal bank account.
- Submit request to: documentcenter@ohionational.com
- Note: funds can only be deposited into a personal bank account.
- Standard processing time of these loans is 5 to 7 business days, but we have been seeing them completed sooner.
Policy Loan Requests under $25k can be requested via phone at 800.366.6654. A bank account must be on file for funds to be electronically deposited. A bank account is on file if premiums are paid monthly or if a previous loan has been requested via paper form with a voided check included.
